If you want your MDI to be equivalent in dosing to your NEB, then you need to give the right number of puffs which I think Justin talked about correctly. Make sure that the patient isn't too sick that they can't use the MDI correctly if they're coming to the hospital. And obviously in the in the ED if they don't look that sick you can start with the MDI. But if they're going to be getting admitted that hopefully means they're relatively sick so start with the nebulizer and then as quickly as possible.
