Is There an Ideal Format to Explain to a Doctor?

For COVID cases, we wear the gowns ourselves. Because in those situations, you'll see it's probably because they have to be sterile. Is there an ideal format to explain to a doctor what's happening with you? We tend to like it in a two types of format. You tell me why it's your main complaint now, like cough. And then tell us in chronology, I started two weeks ago when I was doing this.
