What Are American Pivotal Teeth?

We have a list from the British Dental Association's Museum in London. It is listing American pivot teeth and natural teeth. And you can order odd centrals, laterals, and canines. We don't know how much it would have made this level of dental work available to say someone who was just in the landowning class, but not wealthy.
