Are You Waiting for Failure to Learn?

As the ability to make things safe has improved, we have a higher bar. And so just waiting for things to go wrong means that there is a lot of loss of opportunity to learn. So can you imagine if we just waited for a catastrophic event to happen? We would not be understanding all the weaknesses that are there waiting to combine to turn into event.
