Vasopressin is not going to help your right heart pump better, so I will go to something that is a true that is an ionotrope which vasopressin isn't. If it's somebody with a very stenotic lesion aortic stenosis mitral stenosis or even a right-sided babbular lesion I will typically avoid propofol. Ketamine can actually increase afterload and as long as you do it in a slow titration 10 20 milligrams at a time they will not cause tachycardia.
