On the exam, I think it's either going to be excess-free water or it's going to be SIDH. Or should be diabetic. She could also be diabetic if you think of like a pseudohyponatremia. And then you can get the hyperglycemia from... Hyperglycemia. So how do we make the diagnosis in this hyponatremic patient? We'll calculate their serum-osomes. If they're both low, that's normal and you're talking about excess free water. But if they're in the opposite direction, then you have SIADH. The most likely cause is iatrogenic but there are other treatments
