Matt finds Riley passed out in the park on a merry-go-round thing. He panics thinking she's overdosed and he puts his hand right on the blade of that box. And really fucking cuts himself. His palm is like pouring blood, but he still gets her off the merry- go-round and sets her upright by the bathroom. Then we see Riley regaining a bit of consciousness and she sees the puzzle box on the ground transforming into the next shape. This is right after pinhead has said like, if not, you bring us another.
