The way that themes get built has changed quite a lot in the last five years and even more so in last one year or so. The back end of WordPress already is essentially a headless installation of the editor that just talks through the rest API with the rest of WordPress. So, the front end some projects do use headless approaches, but the majority of them do still build custom themes.
