Ireland is the closest place where they actually do this. They charge, it depends on your particular GP surgery, but it's about 60 euros for a GP appointment. And 100 euros if you turn up at any without a GP's referral letter. The very poor, it's means tested, so the very poorest don't have to pay. But there's always people who are just above that means test level,. and are not rich at all, and are putting put off going to the doctor until it's too late.
