Flight Director Guidance - What Does That Tell You?

The only way that a crew would have caught this locked altitude, even after setting the panel correctly, is to have turned off the flight director and then turned t back on again. And can adjust the barometric reeting before it sat properly. So what does that tell us? Number one, the biggest, most obvious piece here is, why didn't they pull up? What happened there?
