Labatalol is really the drug of choice for aortic dissection because it accomplishes both of our goals, which is lowering blood pressure and reducing inotropy. If you start someone on this and their oxygenation worsens, you need to think that they may be shunting. The dilatory effect interferes with hypoxic pulmonary vasoconstriction, worsens VQ mismatch and leads to poor oxygenation and shunting.
