What Are the Guiding Principles of Care Coordination?
There has to be continuous, 24 seven aces to care. People can't just only receive services when its convenient for us during quotan qote business hours. We have to have more medical and social intigration. There's no one discipline that can actually provide the kind of holistic care that any one person needs. And then finally, there has to be this overriding data driven care coordination structure.
