For identifying occult shock, you can look at the age adjusted shock index that may help. Consider drawing a lactate and a VBG even for older patients who aren't obviously in shock. Clearing the C spine early is of utmost importance since a delay is associated with a whole host of morbidities. For fluids, it's not the end of the world if you give a bit of crystalloid because many of these patients are volume depleted before their injury happened.
