In the majority of cases where there's even any reasonable chance of an upper GI source endoscopy is always the first line with very rare exceptions. When it comes to cirrhosis you would add a couple of things to this list and that is the use of antibiotics, ceptery axon preferred and finding some other equivalent if the patient has an allergy to that or arctriotide infusions instead of PPI.
