The Complexity of Dosing for Gram Negative Bloodstream Infections

Sulfomethoxazole trimethoprimdosing in the setting of infections caused by common gram negatives. The dose recommended in this document is five milligrams per kilogram every 12 hours, which works out to about two double-strength tablets every 12 hours for a 70 kilogram patient. Back-term came out in the 70s and we don't actually have a great PKPD index to target. And so it's not quite as simple, not that any of this is simple, but with some drugs where we at least have kind of a defined PKPD target.
