The Polish Committee for Investigation of National Aviation Accidents
The investigation was carried out by the polish committee for investigation of national aviation accidents. The investigators say that the pro able reason for the landings, one, controller not reacting to the air craft being below the glide path,. And three errors in manual tuning of the radar by the landing zone controller which were caused by unfavorable meteological conditions. So this controller had never really, you know, assisted planes landing in these kinds of conditions. On top of that, this controller had only had nine shifts in the last 12 months, including this one.
