The Psychiatric Shadow Island
The real Rachel turns out to be a hallucination but she tells teddy about her being this acclaimed or respected psychiatrist who was essentially framed so she says by the institutional shadow island. She kind of explains how it can a psychiatrist can manipulate people into being perceived as insane or deranged or whatever and that whenever that label sticks then everything that that patient does becomes an expression of that insanity.
