In acute severe hypertension, hypertensive encephalopathy can present with mean arterial pressures ranging less than 200 millimeters mercury acutely. In chronic severe hypertension, cerebral blood flow is maintained at similar levels as in normal persons but the auto regulatory curve is shifted to the right. Symptoms of press include headache, seizures, vision loss, or even blindness and altered mental status. Dr. Shin: There is no rush to rapidly drop blood pressure in a patient with acute severe hypertension to normal levels. Doing so may actually cause cerebral ischemia.
