Spiro am, to its friends, works by blocking the very important enac or epithelial sudium channels. Acetasolamite is a carbonic and hydrias inhibiter. The drug clearly causes a diaresis and does indeed increase the sodium wasted in the urine. There are a few small trials looking at issus in the ic,. none of which are hugely compelling for a benefit. But i find myself reaching for it when my driven alkalosis is causing issues.
