There have been near misses in the past, haven't there? The Cuban Missile Crisis was a real impulse for both sides to get serious about thinking about how they would control nuclear competition between the Soviet Union and the United States. There's another famous case that comes from the early 1980s when Soviet radars mistakenly is took light reflecting off the clouds in the atmosphere as a launch of American strategic missiles against the Soviet Union.
