Communication Is So Key When Things Go Wrong
Pilot in command and first officer had flown together three times before the incident. Neither pilot nor co pilot had previous experience flying a two member crew aircraft with modern avionics. The standard operating procedure shows that the pilot in command was supposed to be the one taxing to the runway, then transfer controls to the first officer after he's lined up with the runway. Instead, what happened was the increase thrust 55 degrees before lining up, which led to 70 point five percent power being output from the engine.
