The Role of Pharmacoprophylaxis in Postpartum VTE Prevention
There are some guidelines that do call for a pharmacoprophylaxis in patients after vaginal delivery based on morbid obesity. The Royal College of OBGYN states, all women with class three obesity should be considered for prophylactic low molecular weight heparin and doses appropriate for their weight for 10 days after delivery. If the BMI is greater than 40, even though it is a class D recommendation according to the Royal College class D as expert opinion, then yeah, you should consider it. But it's definitely not a hard and fast rule.
