How Do You Write Your Negative Thoughts Down?

Don't put a description of the upsetting event or your feelings in the negative thought column. Instead, put the distorted thought that triggers those feelings. Often people who are anxious will become vaguely aware that there's something upsetting going on. And you'll know your negative thoughts like I'm about to die would cause a panic attack.
