How to Manage a Massive Neck Mass

A 65 year old male presents with a massive neck mass that has been rapidly enlarging over the past two months. On his CT scan, there's a mass effect on the upper airway of this mass with rightward deviation of the trachea. The patient is slightly to kipneck, but at the moment he's breathing comfortably and there's no active strider. He does report that he cannot lie flat at night because of shortness of breath. My initial concern here is his respiratory status.
