The Role of Insulin in Diabetes
One in five of our DKA patients is actually going to be a type 2 diabetic, so it's important that we understand what's going on. In long standing type 2 diabetics, there may be a complete lack of beta cell activity resulting in a lack of insulin secretion. But also some studies suggest that with a constant hyperglycemia resulting from poor control, along with the increased presence of counterregulatory hormones this can cause severe relative insulin deficiency which then can result in the DKA.
