The a Level Algorithm
Exams regulator came up with an algorithm that automatically prescribed what grades students would get. It was designed to counteract the inevitable grade inflation from teacher ascess grades. The algorithm looked at the historical grade distribution of a school and then decided a student's grade on the basis of their class ranking. But if no one from your school had achieved an a star in the past three years, it was extremely unlikely for any one to get one - however brilliant they might be. After a huge public back lash, the exams regulator admitted fault and reverted to teacher ascessgrades.
