In low-income countries, sanitation sometimes is not what we would like. The diet doesn't have enough animal, protein, and animal fat. And that in combination with the low-protein low-fat diet actually leads to a scenario where it used to be called tropical enteropathy. It can also happen in adults, but it's harder to tell because they don't grow as quickly.
