No serious researchers would design a study that way for the reasons Kate describes. So what would they do? First thing you can do is throw in a bunch of controls. Another strategy might be people get eligible for Medicare at age 65. But there are limitations to those quasi experimental strategies. There are also limits to external generalizability.
