The Flight Crew's Decision Making and Crew Resource Management Issues
The training that the flight crew received did not include rejecting a takeoff for tire or wheel failures. Their training on rejected takeoffs only covered engine fire, engine failure and complete electrical failure. The ministry believes that there was also a problem with coordination in the cockpit. There could have been some better cockpit resource management during this accident.
