Intraoperative Management of the Ruptured Aneurysm Case
The only principal difference is the patient who comes in already up-tunded and without a neurologic exam. I personally favor for most of my neuroanesthetics something akin to the old nitrous narcotic approach with a little bit of volatile agent thrown in. However, in that subset of patients in whom I have no hope or plan of reestablishing an exam, I'll typically choose a propofol-based ativa anesthetic. That's what we would transition to in ICU care.
