Is There a Problem Statement?

A problem statement is a minimum combinationf of words and artifects that allows some one to understand your problem. How do i shape the problem statement? It's even weirder about the question is for every different domain you have to do it different. You might be really good at stating a problem in programming, but at talking to your doctor.
