A result tracker is a document or place where you save information about the projects that you're working on. What you specifically did, who was involved, and then the results that came from those projects. You can do this manually if you want. Thereae a whole bunch of ways to do it. The medium doesn't really matter. What matters is the content. And the beautiful part of this is when your performance doview rolls around next quarter or at the end of the year, you have all of the data and all of the specifics from the past in one place.
