In patients without pulmonary hypertension, the cardiac exam will not reveal a loud, single S2 or a hyperdynamic right ventricle like in this patient. Tetralogy of phyllo spells or TET spells are the cyanotic spells typically seen in infants with congenital heart disease and a VSD where the VSD is going to be right to left. Patients who have such mechanical obstruction can have hypoxia, bradycardia and hypoxic arrest.
