First-pass success in this context should really be seen as a system-level quality marker. There will be patients who are absolutely going to benefit in mortality terms from a quick and expertly placed tube so thinking of that obstructed airway or that hypoxic patient. For example those patients who are hematonamically unstable you know is it the first pass that makes the difference to them or is it the impeccable preparation that's made by the team? So for me first-pass success is part of the information you need to look at both in your personal practice and in your service delivery I think it's still important but it's not the be-all and end-all.
