Do I Need to Document the Outpatient Psychiatry?

For those of you who accept managed care, I also want to make a quick note here about insurance requirements for the higher levels of care. Most level of care guidelines stipulate somewhere that there's some kind of collateral contact. They do this because it's best practice. It's not enough to just do those things and you definitely should, but it's also really important to document them.
