Pathophysiology of ACS

The acute coronary syndromes occur when the oxygen demand of the myocardium can't be met by the circulatory system. A reduction in oxygen supply is precipitated by a disruption of an atherosclerotic coronary artery plaque and that results in a clot or a thrombotic response which causes variable obstruction to the flow in the affected coronary artery. And so these plaques that can rupture called fibro lipid plaques are pretty troublesome because they produce pro-coagulant tissue factor and a host of other inflammatory cell mediators.
