How to Use Low Molecular Weight Heparin

If someone is on unfractionated heparin you can stop about 4 to 6 hours prior to the procedure. Poor renal function makes low molecular weight heparin difficult to dose and so people with poor renal function may do better just on a heparin drip. Anyone with a history of hit heparin induced thrombocytopenia you would obviously have to use a non heparin agent like bivalibrutin. And then for resuming post procedure, make sure that there's hemostasis there's not ongoing bleeding.
