Is Mechanistic Knowledge Really Relevant?

The current ranking of evidence is based on internal validity or reliability. But then there is all another question, which is how relevant is this for the patient? So it's possible that a very high rank piece of evidence from the rundermise control trial doesn't tell you much about your patient because irrelevant population was excluded. And at the contrary, you could have evidence from a case series which is not ranked very high in terms of internal validity,. but it could be really just a piece of evidence you need for your patient. We're trying to provide a kind of tool box of systematic thinking to also guide the thinking about external validity.
