What's the Difference Between DAP, Soap, and GIRP?

GIRP is G for goals, S for symptoms, I for interventions, R for response, A for assessment, and P for plan. The DAP format covers the same information regardless of whether or not you consider that D for data or for description. Sometimes it can get a little muddy between what goes in data and what goes in assessment. For example, client was sad versus client appeared tearful.
