The Role of Moral Responsibility in Mental Illness Treatment

In both cases, like the who we should treat is it's going to be the same. It's based on this backward looking fact, not about moral responsibility. But one of them is, let's say, blame worthy and the other maybe isn't. So I'm wondering if that in that kind of case, if yeah, backward looking, deserve blame is going to play a role or not.
