The Intention of Foresight Difference Between Killing Someone and Removing Life Support

The distinction between intention and foresight is relevant not just for issues of medical ethics, it's relevant for many other issues. If we think intentional killings intrinsically evil, does it follow that removing life support that's keeping someone alive is also intrinsically evil? So if we remove life support, we make a judgment that this treatment now is more burdensome than beneficial. But in given human realities, it's almost impossible not to do actions that you foresee are going to cause problems in pain and suffering and even sins.
