I'm most worried about this being a marrow issue. I would probably still send off labs to look for homolysis, just not wanting to anchor. The patient hadn't really been traveling either outside the US or in any of the zones that you mentioned on the endodemic mycosis. So we didn't feel strongly about going down that path. We got a full panel of nutrients thinking that things like B12, folate, even though not likely it would be silly to miss those things as causing something in the marrow.
