Erickson Did Not Tell the First Officer How to Turn on the Lights
The NTSB said that Erickson's inaction with regard to callouts contributed to the breakdown and crew coordination that led to the accident. The first officer should have been calling out altitudes and then letting him know when they got to their minimum altitude, he says. And at the time the airplane began to collide with the trees, the captain was actually telling the first officer how to turn on the lights at the airport.
