The Role of a Carer in a Family
In Agatha Christie's time, caring roles were taken on by the women. Philip feels emasculated in a number of ways and that being an unpaid carer was one of them. Mary seems to define their relationship on the basis that she cares for him. And he needs her as a carer, not that he loves her because she's his partner.
