The Tail Cone Exit System of Northwest Airlines
NTSB: Northwest Airlines maintenance and inspection of the tail cone exit system was inadequate. The interior tail cone release handle was broken when the attempt was made to jettison the tail cone, which is not a good sign. Two people passed away from smoke inhalation in the tail cone; it sounds like they attempted to open that tail cone exit and the handle broke. Was it because of the plane crash? I can't say for certain. If I had to speculate, the handle was probably brittle and probably should have been replaced.
