Back in the day when things went wrong, we wouldn't have really known exactly where that region was involved. We could probably see patterns with this particular outcome and then what happens. You know, like face droopy and problems with speech, but they wouldn't have been able to associate that with a region of the brain as we can now. But I think we did know, or at least for thousands of years, have known that the brain does control much of what we do and think and say and so forth.
