The complications of doing an inner scaling approach include 100% phrenic nerve paralysis. You could also accidentally anesthetize their current layer in geonerve, which would cause the patient to have a horse voice. With someone with COPD, it depends on their clinical state and dependence on their accessory muscles for respiration whether or not you would want to approach this block.
