A positive local wound exploration is when the stab wound has actually violated this anterior rectus fascia. Positive wound exploration means either a transition to the diagnostic imaging or the serial exam pathway. If there are no other indications to go to the OR, patient should be moved into either the serial abdominal exam pathway or the diagnostic imaging pathway.
