How Did They Get Lost?
The captain established the flight path that initially led to the deficiency in situation awareness by mi peting the cali approach controllers cleared to proceed to kali. He changed the flight management system to go direct to the kali v or and removed all the other way points, including tolua. On top of all this that's going on, the flight crew did not realize they also selected the wrong non directional beacon.
