The vast majority of Azure consumers are going to use the pre defined roles, and they probably will use them at a higher level within the kind of hierarchy of non-struct hierarchy subscription resource group resources. From a security like least privilege perspective it's probably best to create custom roles to where you give people only the permissions that they need but nobody does that in practice.
